Male 60 years Cognitive decline with periods of fluctuation Visual hallucinations, H/o falls and parkinsonian features on examination Progressive dementia and died several years later. Macro Brain weight - 1400g Mild gyral atrophy Coronal slices unremarkable Brainstem - pallor of substantia nigra and locus coeruleus
